Why Dental Implants Sometimes Fail - and How to Improve Success

But no procedure is without risk, and being honest about the circumstances under which implants can fail - and what can be done to minimise those risks - is important for anyone considering treatment.
How Long Do Dental Implants Actually Last?
With proper care and maintenance, dental implants can last a very long time - in many cases, decades. The implant fixture itself is designed to be permanent. The crown or restoration on top may need replacing after 15 to 20 years depending on wear and materials. Many patients have implants that have functioned without complication for 20, 25, or even 30 years. The key to longevity is good oral hygiene, regular professional maintenance, and avoiding habits that place excessive stress on the restoration.
Early Failure vs Late Failure
Implant failure falls into two broad categories: early failure and late failure. Early failure occurs during the healing phase, typically within the first few months, when the bone fails to integrate properly with the implant. Late failure occurs after the implant has successfully integrated and can be functioning well for years before problems develop.
Causes of Early Failure
Early implant failure is most often caused by infection, insufficient bone quality or volume, or a failure of osseointegration. Smoking is one of the strongest risk factors - it reduces blood supply to the gum and bone tissue, impairing healing. Patients who smoke are strongly advised to stop before and after implant placement.
Certain medical conditions also increase early failure risk. Uncontrolled diabetes impairs healing and increases infection risk. Bisphosphonate medications used for osteoporosis can affect bone remodelling in ways that complicate implant integration. These require careful assessment and planning.
Causes of Late Failure
Late failure most commonly results from peri-implantitis - an inflammatory condition affecting the gum and bone around the implant, similar to gum disease around natural teeth. Peri-implantitis is largely caused by bacteria accumulating at the implant site due to inadequate oral hygiene. This is why maintaining rigorous cleaning habits and attending regular hygiene appointments is so important after placement.
Excessive biting force is another contributor to late failure. Patients who grind their teeth place significant mechanical stress on implants and restorations over time. A custom night guard can protect both natural teeth and implants in patients who grind.
