Warning Signs You Might Need a Root Canal

Root canal treatment is used to save a tooth when the nerve (pulp) inside becomes inflamed or infected, often due to deep decay, a crack, previous large or deep fillings, or past trauma. Instead of removing the tooth, the dentist carefully cleans and shapes the inside of the root canals, then seals them to prevent reinfection.
A crown is often recommended afterwards to protect the tooth from fracture.
Persistent or Severe Toothache
One of the most common signs that a tooth may need root canal treatment is ongoing pain. This might be:
- A dull ache that does not settle.
- Sharp pain when chewing or biting.
- Pain that is worse when you lie down or at night.
While not every toothache requires root canal treatment, any persistent discomfort should be checked promptly. The earlier the problem is diagnosed, the more options are usually available to save the tooth.
Sensitivity to Hot and Cold
Teeth that react strongly to hot or cold drinks or foods, especially when the sensitivity lingers long after the stimulus is removed, may have nerve involvement. This prolonged sensitivity can suggest inflammation in the pulp, which may not resolve on its own.
Sometimes the tooth can be managed with other treatments, but in many cases root canal therapy is the most effective way to relieve symptoms and preserve the tooth.
Swelling or a Pimple on the Gum
If an infection develops at the tip of the root, you may notice swelling in the gum near the tooth or a small pimple-like bump that may ooze fluid at times. This is a sign that infection is draining and needs prompt professional attention.
Left untreated, the infection can spread and cause facial swelling, significant pain, and generalised illness, as well as tooth loss.
Darkening of the Tooth
A tooth that has suffered trauma or internal damage may gradually darken compared with the surrounding teeth. This colour change can indicate that the nerve has died and the tooth may need root canal treatment.
Sometimes the tooth may not be painful at this stage, but treating the internal infection is still important for long-term health and to allow for cosmetic improvement if appropriate.

Ignoring symptoms can allow infection to spread and may eventually make it impossible to save the tooth. If you notice any of these warning signs, it is important to book a dental assessment.
